2    Go to any Bank near your residence and pay the following:

3    Deposit payment to First Vita Plus corporate bank account in the Philippines.

4.   Scan the documents (see number 5) and send via the following:

5.    Receive a confirmation email from the First Vita Plus Integrated Operations Department to  
       confirm receipt of the following:

1. Dealer's Information Sheet
   
   
2. Security Bank Forms (2)
   
   
3. Deposit Slip / Receipt
   
   
4. Bank-to-bank payment


5. One (1) valid ID Card.

6.    Receive door-to-door delivery of First Vita Plus Power Pack, including an Official Receipt; 
       Security  Bank Cash Card; Acknowledgement Receipts for Shipping and Handling charges  
       and  Security Bank  Cash Card.

7.    Receive sales commissions and sales incentives via ATM, carrying the Cirrus                          
       and/or  Maestro  logos.  Cash deposits withdrawable anytime, anywhere.

8.    Withdraw cash in the currency of the country you are in, anytime!
